To help serve you better, Service Credit Union has provided the forms below for disputes with a charge. If you have a dispute with a charge on your Debit or VISA® cards, please review the list of forms below to file a dispute against the merchants in question. If, however, one of these PDF forms does not “fit” your dispute case, please complete a signed letter instead explaining the full nature of your case.

These letters are intended to assist with what Service Credit Union needs for the dispute process, please be sure to add any additional information or supporting documentation relative to the case since VISA® does not accept a verbal recount of events between the member and the merchant.

or fax to ESD at 603-422-8458 or if you are local to a branch office, feel free to drop the form at a local branch office to send to ESD for processing. You must inform the branch office that this is to be sent to the Electronic Services Department.

***If, you do not receive credit to your account within 5 business days from our receipt of your letter, it is your responsibility to contact ESD at 1-800-936-7730 ext. 3620 for further confirmation on the case.

Unauthorized Charges –
You have never participated nor authorized charges from these merchants. *You must also complete the Lost/Stolen report please.
Lost/Stolen Report -
Please complete this form along with the “Unauthorized Charges” dispute letter as this form is needed for our insurance company.
Non-Receipt of Merchandise or Services –
You haven’t received merchandise/services from a merchant and it’s been 30 days since the order.
Cancellation with Merchant –
You’ve properly cancelled with a merchant and it’s been 30 days since cancellation and the merchant won’t stop charging your account.
Paid by Other Means –
You’ve paid for the same merchandise twice and attempted to work it out with the merchant with no resolution. You must provide proof of payment if paid for by cash, another credit card or other means.
Defective/Damaged merchandise or services –
You’ve received defective/damaged merchandise from a merchant, returned the merchandise via certified mail for proof of return and it’s been 30 days since the return and no credit’s been received from the merchant.
Duplicate Charges –
You’ve been charged on your account twice by a merchant and need to dispute the 2nd charge if the merchant is not willing to assist you.

Verified by Visa

Verified by Visa is a service that is provided by Visa which allows you to register your card with Visa directly for protection with internet purchases. Although it’s not required at this time for all internet purchases, this service is required by some merchants as they also want to protect the cardholder from unauthorized internet services. You can register your Debit or Visa® Card by going to http://www.visa.com/verified and following the steps to register your card.
You can also view places to shop that are verified by Visa merchants.
